The English word "ever" has several distinct uses: in questions about experience, in negative statements meaning "never," in superlatives, in conditional "if ever" patterns, and in emphatic expressions. Japanese expresses these with different grammar patterns and words, not a single equivalent.
Asking if someone has had a certain experience at any point in their life up to now.
Attach to the past plain form of a verb to ask if someone has ever done something. This is the standard pattern for life experiences.
日本に行ったことがありますか。

Casual spoken version dropping が. Common in informal conversation.

Saying that something has not happened at any time.
Literally 'not even once', this strongly emphasizes 'never'. Place 一度も before a negative verb.
彼は一度も遅刻したことがない。
He has never been late.
そんな話は一度も聞いたことがありません。
I have never heard such a story.
Means 'not at all' and often translates to 'never' in context. More casual than 一度も〜ない.
彼女は全然お酒を飲まない。
She never drinks alcohol.
A formal/written adverb meaning 'never' or 'by no means'. Always used with a negative verb.
Emphasizing that something is the most extreme in all of history or one's experience.
Means 'the most ~ so far/ever'. 今まで means 'until now', で marks the scope, and 一番 means 'number one'.
これは今までで一番おいしいラーメンだ。
This is the best ramen I've ever had.
今までで一番高い山に登った。
I climbed the highest mountain I've ever climbed.
Literally 'the greatest in history'. Used for record-breaking achievements or historical extremes.
史上最高の映画だと思う。
I think it's the greatest movie ever.
Means 'for the first time in my life'. Used for personal unprecedented experiences.
生まれて初めて海外に行った。
I went abroad for the first time ever.
Expressing a rare or unlikely condition, often with emphasis.
Literally 'if there is a time when...'. A natural way to say 'if ever' or 'if by any chance'.
もし日本に行くことがあれば、ぜひ連絡してください。
If you ever go to Japan, please contact me.
Means 'in the unlikely event that...'. Stronger emphasis on rarity than もし〜ことがあれば.
万が一遅れたら、電話します。
If I'm ever late, I'll call.
Expressing continuous duration from a point in the past until now, or for all time.
Means 'ever since ~'. Attach to a noun or verb phrase indicating a past point in time.
卒業以来ずっと彼に会っていない。
I haven't seen him ever since graduation.
Means 'eternally' or 'forever'. Used in poetic or emphatic contexts.
愛は永遠に続く。
Love lasts forever.
Means 'forever' or 'for ever and ever'. Common in everyday speech.
Expressing surprise or disbelief that anyone would do something.
一体 adds emphasis to a question, like 'on earth' or 'ever'. Used for rhetorical or surprised questions.
一体誰がそんなことを言ったの？
Who ever said such a thing?
There is no single Japanese word that covers all uses of 'ever'. Using a dictionary translation like かつて or いつか in the wrong context will sound unnatural. Always identify the specific meaning first.
かつて日本に行ったことがありますか。
Have you ever been to Japan? (unnatural)
