Intransitive. Describes a person becoming emotionally unsettled, losing calmness, or panicking. Often used when someone is visibly shaken or unable to think clearly.
He lost his composure at the sudden news.
試験中に取り乱さないように、深呼吸をした。
I took a deep breath so as not to get flustered during the exam.

Transitive. Refers to physically disturbing or disarranging things, such as scattering objects or making a mess. Less common than the intransitive sense.
