お・ ご~申し上げるwe humbly ask, thank, apologize
お願い申し上げます, お詫び申し上げます: the heaviest humble form, for what you or your company do toward a reader or an audience, in formal letters, speeches and announcements.

We respectfully ask for your continued support.
an apology
We deeply apologize for the inconvenience we caused.
thanks
Thank you most sincerely for your many years of patronage.
How to form it
お + Verb ます-stem + 申し上げます ご + する-noun + 申し上げます
願います → お願い申し上げます, 案内する → ご案内申し上げます. In practice, a small set of fixed phrases, nearly always with ます.
The phrases
| Phrase | Says |
|---|---|
| お願い申し上げます | We ask |
| お礼申し上げます | Thank you |
| お詫び申し上げます | We apologize |
| お祝い申し上げます | Congratulations |
| お祈り申し上げます | We wish you (health, success) |
| ご案内申し上げます | We inform you; we invite you |
| お待ち申し上げております | We look forward to seeing you |
Also: 暑中お見舞い申し上げます on a summer card, お悔やみ申し上げます as condolences.
These are for recognizing. Three are worth being able to write, for a formal business mail: お願い申し上げます to close it, お礼申し上げます to thank, お詫び申し上げます to apologize.

The top of three steps
It is the humble お~する at its most formal: you or your side do it, and the reader is on the receiving end.
We ask for your cooperation.
Business mail. Enough for most of it.
We humbly ask for your cooperation.
A formal letter, a notice, a speech, the last line of mail to a client.
In conversation or in mail to a colleague the third is too heavy: it reads like a company notice.

お・ご~するN4
The same humble form at its everyday level, for most verbs with someone on the receiving end.

申す・いたすN4
The humble "say" and "do". いたす makes the middle step above. By itself, 申し上げる is a humble "say" too, but your name stays 田中と申します.
