お・ ご~いただくRespectfully receive or request an action
お・ご~いただく presents a respected person's action as something received by the speaker's side. It is also used for formal, deferential requests.
Whose action is it?
The respected person performs the action, while the speaker or the speaker's group receives it as a favor or benefit. The construction is humble because it describes the event from the receiving side rather than simply raising the speaker's own action.
Receiving a respected person's action
留学の応募にあたり、先生に推薦状をお書きいただきました。
For my study-abroad application, my professor kindly wrote me a letter of recommendation.

Making formal requests
Potential question forms turn the same receiver-side viewpoint into a deferential request. The negative question is often especially restrained.
Choosing the prefix
お is commonly used with native verbs, while ご is commonly used with Sino-Japanese verbal nouns. This is a practical tendency, not a rule that can be applied mechanically: established usage determines which verbs naturally fit the construction.
Compared with the general receiver-side form
お・ご~いただく uses an honorific prefix and is especially common in formal institutional language. The general pattern with a verb's て-form also presents an action as received, but it does not use this honorific prefix-and-stem frame.
担当者に資料を送っていただきました。
The person in charge kindly sent me the materials.
